FUSION meets in the youth center, on the West end of Living Hope Church.

Some FUSION events require medical release or parental permission forms to be filled out. Click the links below to print off medical or parental release forms. MEDICAL/PARENTAL FORM This form must be signed by the parent or guardian in order for the youth to attend certain events. Be sure to fill out the form as accurately as possible. Don’t forget to include emergency contacts and list any allergies or ailments.
